Unlike the Beveridge Model, which is publicly funded, the Bismarck Model relies on multiple, often private, insurance providers to deliver healthcare services, with contributions typically deducted from employees' salaries.

The Bismarck Model originated in Germany with the introduction of the Health Insurance Act of 1883, which was part of Bismarck’s broader social welfare reforms aimed at curbing the influence of socialist movements and solidifying state control. The system was designed to provide health insurance to workers and their families, thus promoting social stability and reducing the appeal of more radical socialist ideologies.

Bismarck’s vision was to create a system where health insurance was mandatory, but also decentralized. Workers were required to contribute to insurance funds, which were managed by non-profit "sickness funds."
